Total triage is being introduced to stop the 8:00am rush to make appointments both by the telephone and attending the practice in person. This new system will ensure that the patients most in need of urgent appointments will be given them.
How it works:-
Monday to Friday between 7:30am to 1:00pm you can access the ‘Triage Form’ on the link above. The form asks a small number of questions in relation to your request. This form is then submitted to be assessed by a care navigator or a clinician depending on your request.
We will then contact you by telephone or text with appointment details, signpost you to the most appropriate care pathway to other healthcare services or advice from the GP.
We must stress that calling the surgery or attending reception will NOT have any advantage over using the online system. Using the form is likely to be quicker to complete than calling or coming to reception. Please note, family members are able to complete the online triage form on your behalf. We understand that there are patients that have limited access to a computer or a smartphone. You can call the surgery for a receptionist to complete the form on your behalf before passing to a clinician or care navigator.
Ensure you submit your request by 1:00pm to ensure your request is dealt with that day. The service is not available at weekends or on bank holidays. For out of hour concerns, please call 111. In the case of an emergency, please call 999.
If you would like to learn how to complete the online form, please come into the surgery and a member of our reception team will be happy to show you how to complete the form for future reference.
We would like to reassure you that these changes are being made to provide better care for our patients and we will continue to strive to deliver a better service. We hope by introducing a total triage system, your experience as our patient will be positive and assure you that these changes are in line with NHS recommendations to provide a modern GP practice model with use of digital technology tools to provide fair access to medical care.

Changes to the GP Contract require all practices to offer and promote the facility for patients to view on-line, export or print any summary information from their records relating to medications, immunisations, test results, allergies and adverse reactions. This is an extension to the online service we have provided to date which allows patients to make appointments, cancel appointments and order repeat medications.
Even if you are a current user in order to use the advanced online items you will have to register for this.
Registration for online services
This practice accepts applications from patients as well as their proxy. Proxy access refers to access to online services by somebody acting on behalf of the patient with the patient’s consent i.e. parent/relative. Proxy access will not be accepted from any third party commercial company i.e. Insurance company or solicitors.
The appropriate application form must be completed prior to any online access being enabled.
Patients requesting online services for the first time or those already registered and would like to request the advanced service.
Please complete the registration form (PDF, 73KB) and attend the surgery and bring with you ONE form of photo ID, this could be a student card, passport or driving licence but must be valid
If patients request access to their detailed coded information they will be notified that it may take the practice up to 10 working days to review their application and grant access if appropriate. This is a guide only and in some circumstances may take longer.
